What is Chronic Care Management?
The Chronic Care Management (CCM) program has been initiated by the Centers for Medicare & Medicaid Services (CMS) as a means to assist patients in achieving improved overall health through coordinated, ongoing care and management of their chronic conditions. These conditions are often of a complex nature making it even more challenging for patients to actively participate in their plan of care. With the use of CCM, your provider will have the opportunity to proactively manage your health-related issues between office visits. How Does CCM Work?
A qualified care manager will help create a care plan and address concerns related to your health. They will also help to identify specific goals, barriers, and outcomes to focus on together. Your care manager will help you to track your progress during the monthly phone call. They will also provide assistance with scheduling appointments, refilling prescriptions, and support self-management.
During these calls, you can address any issues you are facing, or concerns you may have about your health. Your care plan is updated after every call, allowing your provider to monitor your health between office visits. This enables them to address any issues long before a patient may require urgent medical attention or hospitalization. A copy of your care plan is also available for you to share with other care providers that may be involved in other aspects of your well-being.
